Access to SIS

For new or additional access to the Student Information System, you need to request security clearance by completing some or all of these forms:

• SIS: SIS Authorization form
• SIS: Compliance form
• SIS: Imaging System form

Steps to Request Access

This is an accordion element with a series of buttons that open and close related content panels.

I. Complete the SIS Authorization Form

  1. Print the SIS Authorization (opens in new window) form.
  2. Complete the form’s header with your personal information.
  3. Place an ‘A’ for “Add” next to each Role you need.
    Tips:
    – For assistance on what the various roles will grant you, reference the SIS Security Role Catalog (opens in new window).
    – If a subject # is needed, enter Subject Number Code(s) (opens in new window).
    – If requesting access to Student Groups (SR_USG), refer to Student Group Codes (opens in new window).
    – If asking for update access (bold items), access is received after attending training on that topic.
  4. Get all proper required signatures:
    – Yours
    – Dept SIS Authorizer
    – Dean/Director
    Tip: View approved Authorizer List (opens in new window) for appropriate signatures.
  5. Scan and send to security.authorizations@doit.wisc.edu and keep original for your records.

II. Complete the SIS Compliance Form

  1. Print the SIS Compliance (opens in new window) form.
  2. Read the form.
  3. Complete the form’s header information.
  4. Sign the form.
  5. Scan and send to security.authorizations@doit.wisc.edu and keep original for your records.

Note: Only complete this form if you have never had SIS access in your current position.

III. Register for Training

Register for each training course needed.

  1. Go to Learn.
  2. Select ‘Instructor-Led Training’.
  3. ‘Register’ for desired course(s).

Notes:

  • If you ask for update access (bold items), access is received after you attend training on that topic.
  • The SIS Security Role Catalog (opens in new window) describes which training courses correspond with the various role(s).
